Although custom forehead augmentation has become a highly predictable procedure, successful outcomes depend upon thoughtful patient selection, meticulous implant design, precise surgical execution, and realistic patient expectations. Fortunately, true medical complications are uncommon. The majority of revision procedures performed after forehead augmentation are not undertaken because of infection or implant failure, but because of aesthetic issues related to contour, implant design, or patient expectations.
Understanding the distinction between surgical complications and aesthetic dissatisfaction is important. Medical complications generally require prompt intervention, whereas aesthetic revisions are elective procedures undertaken to optimize an otherwise successful result.
Experience demonstrates that most revisions can be avoided through careful preoperative planning rather than intraoperative improvisation.
Infection
Postoperative infection is the most significant medical complication associated with any alloplastic implant. Fortunately, infection following custom forehead implantation is uncommon because of the excellent vascularity of the scalp and forehead.
Most infections occur during the early postoperative period and typically present with increasing pain, progressive swelling, erythema, drainage, or wound separation. Persistent fluid collections or hematomas may increase infection risk by creating a favorable environment for bacterial colonization.
Prevention begins with meticulous sterile technique, perioperative antibiotic administration, careful hemostasis, and elimination of dead space. Equally important is minimizing operative time through efficient implant placement made possible by accurate preoperative design.
When infection occurs, management depends upon its severity and response to antibiotic therapy. Early superficial infections may resolve with antibiotics alone, whereas deep implant infections generally require implant removal, complete resolution of infection, and delayed replacement following adequate tissue healing.
Hematoma and Seroma
Accumulation of blood or serous fluid beneath the implant may interfere with implant stabilization, delay healing, and increase infection risk.
Meticulous intraoperative hemostasis remains the single most effective preventative measure. Limited implant pockets, compressive dressings, and avoidance of unnecessary dead space further reduce the likelihood of postoperative fluid collections.
Most small fluid collections resolve spontaneously, while larger hematomas require prompt evacuation to preserve soft tissue integrity and implant positioning.
Implant Malposition
Because custom implants are designed to match the patient’s skeletal anatomy precisely, true implant malposition has become considerably less common than with earlier stock implants.
When malposition occurs, it almost always results from one of three technical factors:
- incomplete subperiosteal pocket development,
- an oversized implant pocket,
- inadequate fixation.
Incomplete release prevents the implant from seating fully against the underlying bone, whereas excessive dissection permits postoperative migration or rotation.
Proper pocket design remains one of the most important determinants of long-term implant stability.
Palpable or Visible Implant Edges
The forehead provides little opportunity to conceal abrupt contour transitions.
Visible implant borders usually reflect design rather than surgical placement. Abrupt thickness changes, insufficient feathering of implant margins, or inadequate superior and lateral extension create detectable contour irregularities that become increasingly apparent as postoperative swelling resolves.
Modern CAD design software allows implant margins to taper gradually over broad transition zones, effectively eliminating most visible borders.
Treatment depends upon the underlying cause. Localized contour abnormalities occasionally respond to limited implant modification, whereas generalized design deficiencies usually require complete implant redesign.
Contour Irregularities
Because forehead soft tissues closely conform to the underlying implant, contour irregularities represent one of the most common reasons for revision surgery.
These irregularities may result from:
- overprojection,
- underprojection,
- localized implant prominence,
- asymmetrical implant design,
- incomplete implant seating,
- inadequate temporal transition.
Successful correction begins by identifying whether the problem originates from implant design, surgical positioning, or the patient’s native skeletal asymmetry.
Revision without accurate diagnosis frequently reproduces the original aesthetic problem.
Asymmetry
Absolute symmetry is rarely achievable because craniofacial asymmetry exists in nearly every patient.
Minor residual asymmetries should therefore be anticipated and discussed during preoperative consultation.
The objective of revision surgery is improvement of perceptual symmetry rather than elimination of every measurable skeletal difference.
Modern three-dimensional CT analysis greatly facilitates revision planning by allowing direct comparison between implant geometry and underlying skeletal anatomy.
Overcorrection
Among experienced surgeons, overaugmentation is probably the most avoidable aesthetic complication.
Patients often request greater projection than is ultimately beneficial. While it may be tempting to satisfy these requests during implant design, clinical experience consistently demonstrates that excessive projection becomes more apparent over time as swelling resolves.
Because secondary augmentation is technically straightforward, conservative primary design remains the safest long-term strategy.
The old surgical principle remains particularly applicable to forehead augmentation:
“It is easier to add a few millimeters later than to remove several millimeters from an implant that was designed too large.”
Revision Surgery
Revision surgery should never begin with fabrication of a new implant.
Instead, revision begins with repeating the same diagnostic process used before the original operation.
A new high-resolution CT scan allows evaluation of:
- implant position,
- implant design,
- skeletal asymmetry,
- forehead contour,
- temporal transitions,
- brow relationships,
- patient concerns.
Only after the true source of dissatisfaction has been identified should revision implant design begin.
Many revisions require surprisingly small changes. Altering implant thickness by only 1 to 3 mm over selected regions often produces significant improvements in contour while preserving the remainder of the original implant.
Complete redesign should be reserved for patients with generalized aesthetic deficiencies rather than localized contour concerns.
Prevention Through Design
Perhaps the greatest lesson learned from extensive experience with custom forehead implants is that successful revision surgery actually begins before the first operation.
Every design decision influences the probability of future revision.
The most effective prevention strategies include:
- conservative augmentation,
- broad contour transitions,
- adequate superior extension,
- careful temporal blending,
- surgeon-directed implant design,
- precise pocket development,
- stable fixation,
- realistic patient expectations.
When these principles are consistently followed, revision surgery becomes uncommon rather than routine.
Ultimately, the majority of complications associated with forehead augmentation are not failures of implant materials or surgical technique. They are failures of diagnosis, design, or patient selection. Modern three-dimensional planning, patient-specific implant fabrication, and careful aesthetic analysis have dramatically reduced these problems, making custom forehead augmentation one of the most predictable procedures in contemporary facial skeletal surgery.
